directly above! Please note: The next month's schedule is generally posted 2 weeks prior to the start
of the month. If it appears that there are no appointments available, please recheck within 2 weeks of the start
of the month and the available appointments should be visible. If unable

fill out the FAA form 8500-8 (Application for Airmen Medical Certificate) online prior to your visit.
You can do this at https://medxpress.faa.gov . When you submit this form to the FAA website, you will get a confirmation number. You must bring
the confirmation number and a picture ID to your appointment! Dr Schoettmer's offices are in the south
wing of the Jetstream Corporate Center (the runway side). You must park in the general airport parking area
and walk through the pedestrian gate to the Jetstream Corporate Center. Or- just fly in!

Cost: Class 1 or2 Aviation Physical:
$150 Class 3 Aviation Physical $135 EKG with transmittal to FAA: $65 ALL
FIRST TIME EXAMINEES: $50 charge for Color Vision Testing (New Requirement as of 1/1/2025) Initial Special
Issuance exams with document review: $250-350 dependent on condition Follow on Special Issuance medical exam: $175 Actively
flying military pilots: $100 Cash, check or major charge cards accepted. Fees may be higher for special issuance on a case by case basis NOTES: As
of 1/1/2025 thye FAA has changed the color vision requirements/rules. All pilots that have a current medical certificate on
1/1/2025 without color restriction are permanently "grandfathered" and will not need any further color vision testing
going forward. All new applicants must take a computerized color vision test. If this test is failed, the applicant
will recieive a Class III ONLY medical certificate with the restriction "Valid for Day VFR Flight Only". This would
preclude the ability to become a Commercial/Air Carrier Pilot. All of the previously used color vision tests are no longer
valid for new applicants.The previous referral for a light gun test by the FSDO for removal of color vision restriction is
no longer available. The only option for removal of the restriction and the ability to obtain a first or second class medical
certificate is an appeal to the Federal Air Surgeon. Appeals are handled on a case by case basis by the FAA Federal Air Surgeon

Directions to DLZ: From I-270 and
SR 23 on the North side of Columbus: Take SR 23 North 12.9 miles. Turn left under the
"Welcome to Delaware" overpass onto South Sandusky St. and then an immediate right onto SR 42. Go 1.6 miles and
turn right on London Road. The next left is Pittsburgh Drive. The Jetstream Corporate Center is 0.9 miles on the left. From SR 33 and SR 42 on the North-East side of Columbus: Take SR 42 North/East
for 8.4 miles and turn left on Section Line Road. Go 2.1 miles and turn right on Pittsburgh Drive. The Jetstream Corporate
Center is 1.0 miles on the right. OR: Put 1025 Pittsburgh Drive, Delaware,
Ohio 43065 into your GPS!!
